What Is the Medicare GUIDE Program?

The GUIDE Program is a new Medicare benefit launched in 2024 that provides:

  • 72 hours per year of in-home respite care (resets July 1 annually)
  • 24/7 access to dementia-trained registered nurses via PocketRN
  • Care coordination between doctors, specialists, and family
  • Crisis prevention support to avoid ER visits and hospitalizations
  • Caregiver education and emotional support resources

Total cost to families: $0. No copays, deductibles, or out-of-pocket expenses.

This is different from traditional Medicare home health, which requires homebound status and a doctor’s order. GUIDE is specifically designed for dementia care support while your loved one is still relatively independent.

Who Qualifies for Medicare GUIDE Program Near Carol Stream?

  • 72 hours total per year = 18 four-hour visits (minimum visit length)
  • Or 9 eight-hour visits
  • Or 3 full weekend days (24 hours)

Zero cost. The GUIDE Program is 100% covered by Medicare Part A and Part B. No copays, no deductibles, no out-of-pocket expenses for the 72 hours of respite care or 24/7 nurse access. You won’t receive any bills.

That’s completely fine and very common. Many families notice memory problems before seeing a doctor. PocketRN’s nurse will conduct a Medicare-approved cognitive assessment during enrollment. If the assessment shows cognitive decline, your mom qualifies—no separate doctor’s diagnosis required. We’ve enrolled dozens of Carol Stream families this way.

You can use them in any combination, but each visit must be a minimum of 4 hours. So you could do three 24-hour days (72 hours), eighteen 4-hour visits, or any mix. Most families split them up—maybe 4 hours weekly for 12 weeks, then save the rest for emergencies or vacation.

Unfortunately no. The GUIDE Program only works with Original Medicare (Part A and Part B). Medicare Advantage plans (also called Medicare Part C or MAPD plans) are not eligible. If your dad wants to participate, he’d need to switch to Original Medicare during the next enrollment period (October 15 – December 7 annually, or special enrollment if qualifying event).

Your 72 hours reset every year on July 1. If you enroll in November 2025, you get 72 hours through June 30, 2026, then automatically receive another 72 hours July 1, 2026. You keep the 24/7 PocketRN nurse access year-round regardless of respite hours. If you need more than 72 hours of care, Platinum Care Group offers private pay services at our standard DuPage County rates.

Absolutely. You’re in complete control. Want every Tuesday morning? Great. Need occasional full days? Perfect. Planning ahead for a wedding in 6 months? We’ll reserve those hours. You schedule based on your family’s needs, and we match you with available caregivers. Most families book 3-5 days in advance, though we can sometimes accommodate same-day requests.

PocketRN nurses provide guidance and clinical triage, but the decision is always yours. They’ll explain what they’re observing, what it might mean, and whether they recommend ER evaluation, urgent care, calling the doctor in the morning, or managing at home. They can’t diagnose over the phone, but they can help you make informed decisions. In our experience, PocketRN’s triage support has helped Carol Stream families avoid roughly 40% of potential ER visits that turned out not to be emergencies.

No! That’s the beauty of GUIDE versus Medicare home health. The 72 respite hours can give you breaks from daily visits, and the 24/7 nurse access helps coordinate care between you, the assisted living staff, and her doctors.

As of November 2025, we have immediate availability for new GUIDE enrollments in Carol Stream and surrounding areas. The program is very new, so most families haven’t heard about it yet. We can typically complete the entire enrollment process within one week of your first call.

There’s no catch. This is a new Medicare benefit (started July of 2025) designed specifically to support dementia caregivers, who have historically received very little help. Medicare realized that providing 72 hours of respite and care coordination is far less expensive than hospitalizations and nursing home placements. It’s a win-win: families get support they desperately need, and Medicare saves money on crisis care. The only “limitation” is you must have Original Medicare Part A & B, not Medicare Advantage.

Very different. Hospice is for end-of-life care (typically life expectancy of 6 months or less) and requires giving up curative treatment. GUIDE is for people in early to moderate dementia who may live many more years. Your loved one can receive all medical treatments while on GUIDE. If they eventually qualify for hospice, they would transition from GUIDE to hospice at that time.

The 72 respite hours are in-home care for your loved one with dementia, giving you a break. They work together as a complete support system.
